Early Intervention Service Coordination Program for Children Birth to Age Three With Developmental Delays and Disabilities

Under contract with the City of Philadelphia's Department of Behavioral Health and Intellectual disAbility Services (IdS), PHMC administers the ChildLink/Philadelphia County Program. ChildLink/Philadelphia County provides Service Coordinators who help children, from birth to age three who have developmental delays or disabilities, and their families living in Philadelphia obtain supports and services. ChildLink/Philadelphia County staff work in partnership with parents and early intervention specialists and therapists to evaluate each child's needs, identify outcomes, explore options and develop an Individualized Family Service Plan (IFSP) composed of supports and services for each eligible child and family. ChildLink/Philadelphia County services are provided at no cost to families.

ChildLink/Philadelphia County provides the following services for all children, birth to age three, who reside in Philadelphia:

• Arrange for evaluations to determine if a child is eligible for early intervention services
• Develop an IFSP to meet the individual needs of eligible children and their families
• Coordinate and obtain early intervention supports and services needed by eligible children and their families
• Monitor supports and services and facilitate reviews and changes to the IFSP
• Assist eligible children to transition at age three to pre-school early intervention and/or other community and specialized supports and services
• Monitor children until age three for their development if they are at-risk for a developmental delay due to medical or environmental factors

Health Intervention Program (HIP)

HIP is a home visiting and case management program for families who want support and resources related to caring for a child with a special health care need, ages birth to 21 years. Services are provided by a team of a nurse, social worker, a parent-professional (who is also the parent of a child with special needs) and a substance abuse counselor. We can help with:

• Health care and insurance — helping to obtain and coordinate care
• Supportive health needs — dental care, lead safety, and immunizations
• Educational needs — working with the schools and service providers
• Healthy lifestyle information — smoking, nutrition, stress, recreation
• Home safety — making the home child proof and accessible
• Referrals for all kinds of help — utilities, child care, food and more
• Fun parenting and health education workshops
