ChildLink - Philadelphia CountyEarly Intervention Service Coordination Program for Children Birth to Age Three With Developmental Delays and Disabilities Under contract with the City of Philadelphia's Department of Behavioral Health and Intellectual disAbility Services (IdS), PHMC administers the ChildLink/Philadelphia County Program. ChildLink/Philadelphia County provides Service Coordinators who help children, from birth to age three who have developmental delays or disabilities, and their families living in Philadelphia obtain supports and services. ChildLink/Philadelphia County staff work in partnership with parents and early intervention specialists and therapists to evaluate each child's needs, identify outcomes, explore options and develop an Individualized Family Service Plan (IFSP) composed of supports and services for each eligible child and family. ChildLink/Philadelphia County services are provided at no cost to families. Health Intervention Program (HIP)HIP is a home visiting and case management program for families who want support and resources related to caring for a child with a special health care need, ages birth to 21 years.
