HIV Care Access ProgramThe goal of the HIV Care Access Program is to serve individuals living with HIV who are not currently participating in the HIV system of care and are lost to follow-up medical treatment. PHMC Care Access Field Specialists partner with medical providers to conduct case finding and locating activities in the community and assist consumers to reconnect to the continuum of HIV medical and case management services. Home and Community Based HIV Services ProgramThe Home and Community Based HIV Services Program has been administered by PHMC since 1992. Home health care services are an integral part of the continuum of care of services provided to persons with HIV disease and may be required at any stage of the illness. PHMC subcontracts with Keystone Home Care, Inc. a Medicare Certified provider, who renders the following home-based services to eligible clients of this program: Skilled Nursing, Home Health Aide Care, Home Intravenous Drug Therapy, Durable Medical Equipment, Disposable Medical Supplies and Physical Therapy. To qualify for the program, a client must be within 300% of the poverty level, live within one of the five Philadelphia metropolitan counties (i.e. Bucks, Chester, Delaware, Montgomery, or Philadelphia), and be uninsured, underinsured, or have exhausted his or her insurance benefits. By supplementing the care that insurance reimburses, the Home Health Care Program has often been instrumental in bridging gaps in services while patients await insurance approval. Most individuals with AIDS are poor and must rely on the limited benefits of public health insurance and usually cannot afford to purchase additional services when they are needed. Medicaid affiliated health maintenance organizations can be used to meet individuals' needs for home-based care, but many times the wait to access these services is lengthy, especially if the patient has no other resources and is applying for initial insurance coverage. In other situations, when patients require more services than their insurance benefits will allow or when their home care benefits have been exhausted, the Home Health Care Program provides services to ensure that all of the patient's clinical home-based needs are met. PHMC Care ClinicPHMC now managing Care Clinic at St. Joseph's Hospital. Since June 2005 the HIV/AIDS Care Clinic, formerly managed by St. Joseph's Hospital, has been a part of the Public Health Management Corporation. The Care Clinic, a provider of quality community-based primary care to the HIV/AIDS community since 1989, primarily assists residents of North Philadelphia who are insured through Medical Assistance.
