HIV Care Outreach

Home and Community Based HIV Services Program

The Home and Community Based HIV Services Program has been administered by PHMC since 1992. Home health care services are an integral part of the continuum of care of services provided to persons with HIV disease and may be required at any stage of the illness. PHMC subcontracts with Keystone Home Care, Inc. a Medicare Certified provider, who renders the following home-based services to eligible clients of this program: Skilled Nursing, Home Health Aide Care, Home Intravenous Drug Therapy, Durable Medical Equipment, Disposable Medical Supplies and Physical Therapy. To qualify for the program, a client must be within 300% of the poverty level, live within one of the five Philadelphia metropolitan counties (i.e. Bucks, Chester, Delaware, Montgomery, or Philadelphia), and be uninsured, underinsured, or have exhausted his or her insurance benefits.By supplementing the care that insurance reimburses, the Home Health Care Program has often been instrumental in bridging gaps in services while patients await insurance approval. Most individuals with AIDS are poor and must rely on the limited benefits of public health insurance and usually cannot afford to purchase additional services when they are needed. Medicaid affiliated health maintenance organizations can be used to meet individuals' needs for home-based care, but many times the wait to access these services is lengthy, especially if the patient has no other resources and is applying for initial insurance coverage. In other situations, when patients require more services than their insurance benefits will allow or when their home care benefits have been exhausted, the Home Health Care Program provides services to ensure that all of the patient's clinical home-based needs are met. HIV Care Outreach and Retention Program

The HIV Care Outreach and Retention Program serves individuals who are living with HIV, are not currently participating in the HIV system of care, and are lost to follow-up medical treatment. HIV care outreach specialists partner with medical providers to conduct locating and follow-up activities in the community and assist individuals to reconnect to the continuum of HIV medical and case management services. This program is supported by the AIDS Activities Coordinating Office (AACO) of the Philadelphia Department of Public Health.

Philadelphia Hepatitis Outreach Project

Philadelphia Hepatitis Outreach Project (P-HOP) conducts community-based hepatitis prevention education, screening, testing and linkage to medical care and support services. Outreach services are implemented in substance abuse treatment and public housing facilities, HIV early intervention programs, homeless shelters among others. Additionally, project staff conduct and participate in community health fairs which provide neighborhood residents with viral hepatitis-related health information and the opportunity to receive HCV counseling and screening services. Follow-up events are scheduled for each health fair to provide residents with their HCV screening results and facilitate referrals to confirmatory testing services for those who screened positive for HCV. All services are available in Spanish and English. This project is supported with funding from the Office of Addiction Services of the Philadelphia Department of Behavioral Health and Mental Retardation Services.

PHMC Care Clinic

The Care Clinic provides quality, community-based primary care to adults requiring specialized health care and social services. The PHMC Care Clinic opened in 1992 as a primary care and specialized health services clinic under the guidance of St. Joseph’s Hospital in North Philadelphia. In 2005, St. Joseph’s turned over the management of the clinic to Public Health Management Corporation (PHMC), then The Philadlephia Health Management Corporation, a nonprofit, public health organization that had more than 30 years of experience running direct service programs.

PHMC Care Clinic is a Federally Qualified Health Center (FQHC) and participates in a Section 340B pharmacy purchasing agreement with Coordinated Care Network.

Our Clients

The PHMC Care Clinic services the special health needs of the community of North Philadelphia, which has a disproportionately high rate of unmet healthcare needs. We offer medical and support services and case management to each of our patients.

Our Staff

Our bilingual staff includes doctors, physician assistants, case managers, a medical assistant and treatment adherence counselors who strive to ensure our patients receive the highest level of care.
