PHMC Health Care Centers are located throughout the city of Philadelphia and offer high quality health care to Philadelphia residents of all ages. PHMC's growing network of nurse-managed health centers includes the Mary Howard Health Center, Philadelphia's primary care center for the homeless, Rising Sun Health Center, a health center in Northeast Philadelphia, Project Salud, a Chester County health center that is part of PHMC affiliate La Comunidad Hispana, and Community Court which provides services to the clients of Community Court. PHMC's health centers provide comprehensive primary health care, family planning, disease management, and social services to men, women and children.
