It’s National Health Center Week, August 13-19th, and time to celebrate the critical role America’s health centers play in building healthier communities. PHMC’s six nurse-managed, Federally Qualified Health Centers assist underserved communities, particularly public housing populations and individuals experiencing homelessness, by providing high-quality, patient-centered primary care throughout the region.

As the opioid crisis continues, PHMC health network offers the PHMC Center of Excellence (COE), a model of coordinated care that uses a Community Based Care Management (CBCM) team of social workers and care managers to help individuals experiencing opioid use disorder (OUD) access a variety of services to support them in their recovery. The CBCM team helps ensure that those with opioid use disorder do not fall out of care. The team assists patients in developing care plans and connecting to resources and treatment options.
