Mill Creek School
Under contract with the City of Philadelphia's Department of Behavioral Health and Intellectual disAbility Services (IdS), PHMC administers the ChildLink/Philadelphia County Program. ChildLink/Philadelphia County provides Service Coordinators who help children, from birth to age three who have developmental delays or disabilities, and their families living in Philadelphia obtain supports and services. ChildLink/Philadelphia County staff work in partnership with parents and early intervention specialists and therapists to evaluate each child's needs, identify outcomes, explore options and develop an Individualized Family Service Plan (IFSP) composed of supports and services for each eligible child and family. ChildLink/Philadelphia County services are provided at no cost to families.

PersonLink is one of the agencies in Philadelphia that provides supports to individuals with a diagnosis of intellectual disabilities. PersonLink contracts with the Commonwealth's Department of Public Welfare Office of Developmental Programs. The program offers supports coordination services city-wide to persons with intellectual disabilities and operates the Special Offenders Program, an initiative that provides supports coordination to individuals with intellectual disabilities involved with the criminal justice system.

The Mill Creek School is a secondary educational program offering a comprehensive academic curriculum for high school students. It is fully licensed by the Commonwealth of Pennsylvania and the State of New Jersey for grades 7 - 12. The school offers a full academic schedule of classes in which each student has an individualized program worked out with his/her Mill Creek advisor. Academic credits earned at Mill Creek are honored by the school districts from which our students come and the schools they leave us to attend.
